How to fill out:   Click the form you need to download then fill in the blanks within the word document, click "File" and then "Save as" in the top left corner of the Word App, save the file as your first name and last initial (for example, "Sara T"), and then attach the file to an email and send to certifiedhandtherapy@gmail.com prior to your first appointment.
