Plan for your first visit

A physician referral is not required to begin treatment at our clinic and we accept the following insurances: Aetna, Anthem/BCBS, Cigna/ASH, Medicaid, Medicare, and Optum/United Healthcare. Your new patient paperwork will get emailed to you once you contact our office to schedule your initial evaluation.
